Actually, it begins with the cheaper steel heater elements that are used in some cheaper tubs. Most of the better brands use a coated element so the steel is not directly exposed to the chemically treated water. Add poor chemical maintenance to the mix (pH issue, excessive sanitizer and/or oxidizer, water hardness issue, etc.) and you have a recipe for rapid heater corrosion. It's not helpful that you also have two dissimilar metals in proximity to each other which adds electrolysis to the mix.
